*Release of Liability: I am the parent/legal guardian of the player submitting this registration form. By submitting the preregistration, I consent and agree that the player may participate in tryouts and if selected and signed to a team, participate as a part of Pato's Magic Soccer Club and the leagues they affiliate with. I agree to release, hold harmless, defined and indemnify Pato's Magic Soccer, affiliated leagues & fields and all those acting on their behalf in any capacity on account of all claims, demands & damages sustained by the player or player's property resulting directly or indirectly from participation, practice and play in any Magic Soccer open house & tryouts programs or activities. We further certify that the player is not registered or signed for next season year by a US Youth Soccer sanctioned organization. We acknowledge that we understand that this is an open house & tryout and attendance does not guarantee selection. I further attest that I am attending of my own free will and have not been approached or offered a position on any Magic Soccer team prior to this tryout. I agree to make any payments on time and understand that all fees are nonrefundable. I authorize Magic Soccer to use Photos of my player/family on their website.
